PhilHealth YAKAP Registration 2026: Requirements and Step-by-Step Guide

PhilHealth YAKAP Registration 2026: Requirements and Step-by-Step Guide

Quick Answer: To register for PhilHealth YAKAP you need an active PhilHealth PIN, then you pick a YAKAP clinic through eGovPH, the PhilHealth Member Portal, any PhilHealth office, or at the clinic itself. But choosing a clinic is only half of it — registration is not finished until you attend your First Patient Encounter (FPE) at that clinic and sign the YAKAP Empanelment Slip. That signature is what entitles you to continuous care, and it also determines whether you can switch clinics later. Registration is free. At a public YAKAP clinic you should never be charged for covered services; at a private one your co-payment is capped at ₱900 per member per year.

About this guide: Sourced from primary PhilHealth issuances — Circular 2025-0017 (selection and empanelment, signed 13 October 2025), Advisory 2026-0003 (member rights, 8 January 2026), Advisory 2025-0065 (clinic transfer, 2 December 2025) and Advisory 2026-0007 (medicines, 14 January 2026). Processes and portal screens change — confirm current steps with PhilHealth or your clinic before travelling to an office. Last checked: August 2026.

What "registering for YAKAP" actually means

YAKAP — Yaman ng Kalusugan Program — is PhilHealth's primary care benefit, the programme formerly called Konsulta. Registering for it is not a single form. It is two distinct steps, and most people who think they are registered have only done the first.

Step one is selection: you choose one YAKAP-accredited clinic as your primary care provider. Step two is empanelment: you actually show up at that clinic for your First Patient Encounter, and sign the YAKAP Empanelment Slip. Until step two happens, you have expressed a preference but you are not yet empanelled, and the clinic has no obligation to provide you continuous care.

The legal basis for the whole arrangement is the Universal Health Care Act, which PhilHealth Circular 2025-0017 quotes directly: "Every Filipino shall register with a public or private primary care provider of choice" (Chapter II, Section 6.d). The circular exists specifically to protect your right to choose which clinic that is.

If you're still unclear on what the benefit itself covers, read our PhilHealth YAKAP benefits guide first, then come back here for the mechanics.

Requirements: what you need before you start

The list is short. YAKAP registration is not restricted to a particular membership category — any PhilHealth member with an active PIN can register, and so can qualified dependents.

What you needNotes
An active PhilHealth PINIf you don't have one yet, get it first — see PhilHealth ID requirements and our online registration guide
Identity verificationDone through the PhilHealth Check Utility (PCU), which includes a liveness check
A valid government-issued IDFallback if PCU verification can't complete — for example if your PhilSys registration isn't finalised
A chosen YAKAP clinicPublic or private; the choice is yours and it changes what you pay

You do not need to pay anything to register, and PhilHealth's own guide says the First Patient Encounter has no fee at accredited clinics. If you were already enrolled under the old Konsulta name, you do not start from zero, but check your status. PhilHealth renewed members who had a First Patient Encounter in 2025 to the same clinic for 2026, and members sign the YAKAP Empanelment Slip again each year. Members who were assigned a clinic but never had a First Patient Encounter in 2025 were not renewed and need to select a clinic again (Advisory 2026-0019).

One thing worth checking before you commit to a clinic: PhilHealth now requires YAKAP providers to operate a fixed, permanent physical clinic, with limits placed on mobile and telemedicine-only providers for accreditation and empanelment purposes. A pop-up or purely online outfit offering to empanel you is a reason to pause and verify.

The four ways to register

PhilHealth YAKAP: Primary Care, GAMOT and Screening documents the Member Portal clinic-selection flow alongside the rest of the program.

1. The PhilHealth Member Portal

Log in to the Member Portal with your PIN, choose Click here to Select YAKAP Clinic, search by province or city or municipality, and pick your clinic. For a dependent, scroll to the dependents list, find the YAKAP Clinic column beside their date of birth, and select a clinic the same way. These are the steps PhilHealth listed in Circular 2025-0017, so the screens may look a little different now. This is the most direct route if your PhilHealth online account already works. If you need to sort out your member record first, our MDR online guide covers that.

2. eGovPH

In the eGov PH app, go to NGAs, search for PhilHealth, open PhilHealth Member Portal, then use the YAKAP tab and Add YAKAP Clinic for yourself or a dependent. These are the steps PhilHealth listed in Circular 2025-0017, so the screens may look a little different now. Useful if you already have eGovPH set up for other government transactions and would rather not maintain a separate PhilHealth login.

3. At a PhilHealth office

Any PhilHealth Local Health Insurance Office (LHIO) or Regional Office can process your registration in person. Bring a valid government ID. PhilHealth's transfer guidelines tell you to file a clinic transfer at an LHIO, so it's worth knowing where your nearest office is — see our PhilHealth offices directory.

4. Directly at a YAKAP clinic

Often the most efficient option, because a clinic can frequently handle both your selection and your First Patient Encounter in one visit. You walk in unregistered and leave fully empanelled, instead of registering online and then making a second trip. If you already know which clinic you want, this is usually the fastest path to actually being covered.

The First Patient Encounter: what completes your registration

The FPE is the visit where your registration becomes real. Practically, it involves identity verification and a liveness check through the PhilHealth Check Utility, an initial assessment by the clinic, and your signature on the YAKAP Empanelment Slip.

Get the slip signed. It matters more than it looks:

  • It triggers your right to continuous care. PhilHealth's member advisory states you have the right to continuous and appropriate primary care from your empanelled clinic once you have completed your FPE and signed your YAKAP Empanelment Slip.
  • It sets the rules for switching clinics. Whether you can transfer freely or only in the fourth quarter depends entirely on whether your FPE has happened — see below.

Since May 2026 PhilHealth has required clinics to use a digital empanelment slip, where a second PhilHealth Check Utility liveness check works as your signature. A printed copy is available if you ask, so ask for one. The slip covers one calendar year, so you sign it again every year.

How much YAKAP registration costs

Registration itself is free. What can cost you is the type of clinic you pick, and PhilHealth put the rules in writing in a member advisory titled Know Your Rights Under the YAKAP Primary Care Benefit, signed by President and CEO Edwin M. Mercado on 8 January 2026:

"If you are receiving YAKAP primary care services in a public YAKAP clinic, you should not be charged any fee."

"your out-of-pocket co-payment is limited to a maximum of P900 per member per year… Once you have reached this P900 annual co-payment cap, all remaining covered and necessary YAKAP services for the year should be provided to you at no additional cost."

So: public clinic, nothing. Private clinic, ₱900 a year maximum — a ceiling on the total for the whole year. PhilHealth Advisory 2025-0057 lets a private clinic collect it per visit (for example ₱150 per consultation) or as one payment at enrolment or your first visit, as long as the year's total stays within ₱900. Once you've paid ₱900 in total, the rest of that year's covered services should cost you nothing more.

A separate figure you'll see quoted, ₱20,000, is not a cap on your registration or on the programme — it is the annual limit of the GAMOT medicines benefit. Your YAKAP clinic also dispenses 21 essential medicines directly, including amoxicillin, paracetamol, salbutamol, metformin, losartan and amlodipine. The full breakdown is in our free medicines from PhilHealth guide.

How to switch your YAKAP clinic later

This is where the FPE comes back. Under PhilHealth Advisory 2025-0065, transfer rights split into two cases:

Your statusWhen you can transferWhen it takes effect
FPE not yet availedAny time of the yearImmediately upon approval
FPE already availedFourth quarter only (October–December)1 January of the following year

To file, visit any PhilHealth LHIO and undergo member verification and a liveness check through the PCU. If PCU verification can't be completed — for instance because your PhilSys registration isn't finalised — you may instead present one valid government-issued ID (or acceptable alternative document) together with a duly accomplished PhilHealth Konsulta Registration Form (PKRF).

Someone can file on your behalf, but they must bring an authorisation letter from you plus the proof of identity above. You'll get an email confirming your assignment to the new clinic.

The practical takeaway: choose carefully the first time. Once you've had your FPE, you're with that clinic until at least the following January, unless your clinic closes, loses its accreditation, or is ordered to stop operating by the DOH. PhilHealth allows an earlier transfer in those cases.

If you're charged or refused service

You have three named rights under Advisory 2026-0003: not to be charged in a public YAKAP clinic, not to pay more than ₱900 a year in a private one, and not to be denied primary care once empanelled.

If any of those is breached, PhilHealth's 24/7 Corporate Action Center is the escalation route:

PhilHealth has also addressed the excuse you're most likely to hear. In Advisory 2026-0004 (8 January 2026) it responded to reports of YAKAP system outages being used to justify charging patients, stating that its systems are operational and that "members should not be required to pay for YAKAP primary care services related to the said system interruption."

Frequently Asked Questions

What are the requirements for PhilHealth YAKAP registration?

An active PhilHealth PIN, identity verification through the PhilHealth Check Utility (which includes a liveness check), and a valid government-issued ID as fallback if PCU verification can't complete. You also choose a YAKAP clinic. There is no fee to register.

How do I register for PhilHealth YAKAP?

Four channels: the PhilHealth Member Portal (YAKAP tab, then "Add YAKAP Clinic"), the eGovPH app, any PhilHealth office, or directly at a YAKAP clinic. Registering at the clinic is often fastest because many can do your First Patient Encounter in the same visit.

Is YAKAP registration free?

Yes. Registration costs nothing. Separately, covered services are free at public YAKAP clinics, while private YAKAP clinics may collect a co-payment capped at ₱900 per member per year.

Am I registered once I pick a clinic online?

Not fully. Selecting a clinic is step one; you are only empanelled after your First Patient Encounter at that clinic and after signing the YAKAP Empanelment Slip. Your right to continuous care starts at that point.

Do I need to register again if I was enrolled in Konsulta?

Not from scratch, but check your status. YAKAP is the expanded Konsulta. PhilHealth Advisory 2026-0019 says members who had a First Patient Encounter in 2025 were renewed to the same clinic for 2026, while members assigned a clinic without a 2025 First Patient Encounter were not renewed and must select a clinic again. Everyone signs the YAKAP Empanelment Slip for the new year. If you're unsure of your status, confirm with your assigned clinic or a PhilHealth office.

Can I change my YAKAP clinic after registering?

Yes, but timing depends on your FPE. No FPE yet means you can transfer any time, effective immediately. If you've already had your FPE, you can only request the transfer in October–December, effective 1 January. File at any PhilHealth LHIO.

Can someone register or transfer on my behalf?

For transfers, yes — your representative needs an authorisation letter from you plus valid proof of identity. Your own presence is required for the First Patient Encounter itself, since it includes a liveness check.

What if the clinic says the YAKAP system is down and asks me to pay?

PhilHealth addressed this directly in Advisory 2026-0004, stating its systems are operational and that members should not be required to pay for YAKAP primary care services because of a system interruption. Report it to yakaptugon@philhealth.gov.ph or (02) 866-225-88.

Conclusion

The part people get wrong is assuming that picking a clinic online means they're registered. It doesn't — the First Patient Encounter and the signed Empanelment Slip are what actually turn a selection into coverage, and they're also what locks you to that clinic until the following January. So choose the clinic you'll realistically travel to, and do the FPE promptly.
